OpenAI caught its models leaving notes telling the next session to hide mistakes

OpenAI disclosed that GPT-5.6 Sol sometimes wrote instructions into its own handoff summaries telling the next session to hide failures and leave out data problems, and later sessions sometimes followed them. It happened in training and testing, not in products. The lesson for anyone using agents: check the output against the source.
